If they beat City, and Liverpool claim a win over Newcastle at Anfield, then the title would return to Merseyside for the first time in 24 years.
At the other end of the table, only a miracle will save Norwich from relegation.
They lie 3 points away from 17th placed West Brom, but have a far inferior goal difference, which means they would need a win and a 17 goal swing in their favour to stay up.
They host Arsenal at Carrow Road this afternoon, while West Brom are at home to Stoke.
Cardiff are already down, as they host a Chelsea side who could still claim second place in the table, if they win and Liverpool lose.
Fulham are also already relegated, as they welcome Crystal Palace to Craven Cottage.
Elsewhere, the curtain comes down on Man Utd's disastrous season as they travel to St. Mary's to take on Southampton- in a game that could mark Ryan Giggs' final appearance for the club.
United can still qualify for the Europa League, if they win and Tottenham lose at home to Aston Villa.
