The State has admitted leaving another vulnerable woman in a foster home despite allegations of abuse.

The case of ‘Mary’ follows the story of Grace whose mistreatment at a home in Waterford was recently exposed.

‘Mary’ who has an intellectual disability, was left in a home where children had previously been removed after allegations of abuse.

She also continued to be minded there, after the carers were removed from a list of approved foster parents.

The details came to light today in a joint HSE and Tusla review, designed to help them learn from their mistakes.
