The British Maritime and Coastguard Agency is to review the circumstances surrounding the death of Caitriona Lucas last September.

The 41-year-old Doolin Coast Guard member became the first Irish Coast Guard volunteer to lose their life in service following an incident off Kilkee during a search operation there last September.

Investigations are already being carried out by The Marine Casualty Investigation Board, and by the Health and Safety Authority.

The Irish Times reports that the Irish Coast Guard has requested the British review, after it said it would undertake an internal inquiry.

This probe is expected to focus on procedures, training and the events leading up to the tragedy.
