A patient lost his life – and two paramedics were injured in the incident – which broke out at the entrance to the hospital’s Emergency Department.
The HSE is looking into the possibility that oxygen tanks on the vehicle may have caused ‘a small explosion’ and a notice has been sent to ambulance crews across the country.
SIPTU Health organiser Paul Bell says a full investigation of the fleet must now take place.
