Ennis Chronic Disease Management Hub To Become Operational On Phased Basis From Next Week

A Chronic Disease Management Hub is to become operational in Ennis on a phased basis from next week.

The facility is to provide care to those with cardiovascular disease, respiratory illnesses and type 2 diabetes.

The hub will be located on Station Road within the current Ennis Primary Care Centre.

 

It’ll provide pulmonary rehabilitation services, help with diabetes self management, diagnostic services for chronic disease, and a phased introduction of cardiac rehabilitation assistance.

In correspondence to Clare Independent TD Michael McNamara, seen by Clare FM, the HSE indicates the facility is ‘anticipated’ to become operational before the end of this year.

20 whole time equivalent staff have been assigned to the hub, with eight of those already in post.

These include the Operational Lead and two Clinical Nurse Specialists, with recruitment processes for a Senior Dietician, a Senior Physiotherapist and Respiratory and Cardiology Clinical Nurses understood to be at an advanced stage.

The HSE says it’s ‘difficult’ to say when the full Chronic Disease Team will be in place, with additional posts in the region being recruited for through the UL Hospitals Group.

Progammes to empower those living with a chronic disease to manage their condition are planned for Kilkee, Ennis and other locations in the county throughout 2023.
