The health minister Leo Varadkar says if the new patient safety measures he's announcing today work, then adverse events like those which sparked the reforms won't happen.

A package of measures including plans for a new support service, an advisory council and an annual patient survey, is being introduced.
It was developed in response to high profile cases in recent years, including the death of Savita Halappanavar and last year's report on baby deaths at Portlaoise hospital.
Minister Varadkar says the reforms are aimed at preventing adverse events like these from happening and avoiding patients and their families having to go to court.
