A couple have received an unreserved apology after losing two babies at Portiuncla hospital.
Lorraine and Warren Reilly from Galway lost their two baby girls in 2008 and 2010 over failings at the maternity hospital.
The couple say they were never informed that the hospital was carrying out a review into infant care at the facility.
