The official HIQA report into the death of Savita Halappanavar was published yesterday
It found a series of failures in the provision of basic medical care and that staff in University Hospital Galway failed to recognise Mrs Halappanavar’s infection and deterioration
The report went on to say there were a number of missed opportunities which, if acted upon, may have changed the outcome of her care

On Thursday’s Morning Focus we heard HIQA Director of Regulation Phelim Quinn speaking at yesterday’s publication launch

The report makes 34 recommendations for maternity services both in Galway and nationwide which Petrina Vousden, Health Editor with the Irish Daily Mail, outlined to John Cooke.

Kilmaley resident Simon McNamara contact Morning Focus to say that while walking his dog on Tuesday last week he noticed a dead Badger on the road
The badger carcass remains on the roadside and Simon wanted to know who is responsible for its removal.
